Person-Centered Planning and Independent Facilitator

Maliaka & Associates is rooted in the principles of rights, independence, and choice. Person-Centered Planning is a process directed by the family or the individual-served, intended to identify the individual’s strengths, capacities, preferences, needs, and desired outcomes.  A person-centered plan provides information about a person.  This process helps individuals work out what they want, find the support they require and help get it. This plan is a process with the person at the center of the plan family and friends are partners in Planning. Proper Planning leads to positive change.

Maliaka & Associates believe that person-centered Planning is an ongoing process.  Person Center Planning leads to continual operation of listening, learning, and action and helps the person gets what they want out of life. Understanding from Planning can inform individuals and select service delivery as a whole and inform and inspire others to achieve more extraordinary things. It is about citizenship, inclusion in family, community, the mainstream of life, and self-determination. This approach requires some very fundamental changes in thinking and the established balances of power, the implications of which are potentially enormous and far-reaching.

Person-Center Planning with an Independent Facilitator is a service facilitated by a certified independent facilitator, directed by the individual-served or the family, to identify the individual’s strengths, capacities, preferences, needs, and desired outcomes. An assessment is a tool used to elicit information about the individual-served. The collected information creates the Individual Plan (Person-Centered plan).
